• RN - Licensed Registered Nurse NJ - State of New Jersey or RN - Licensed Registered Nurse PA - State of Pennsylvania.

Responsibilities

• Interacts with co-workers, visitors, and other staff to support patient care.
• Advocates for the patient and family throughout the entire episode of care and facilitates communication among the team.
• Facilitates patient progression throughout the patient’s hospital stay.
• Reviews length of stay and barriers data to support discharge planning.
• Monitors progress and resolves barriers to discharge in a timely manner.
