Job summary

Enhabit Home Health & Hospice in Norwood, MA is seeking an experienced healthcare professional to assist patients navigating post-acute care with patient-centered transition plans and adherence to evidence-based guidelines.

You will collaborate with health systems and physicians, ensure compliance with Medicare regulations, and contribute to care quality while enjoying generous benefits including PTO, 401(k) matching, and professional development opportunities.

Qualifications

• Graduate of an approved school of nursing, therapy or social work.
• Licensed in the state where they practice.
• Two years' demonstrated field experience.
• Knowledge of federal, state, and local laws and regulatory guidelines for Medicare home health and hospice.
• Basic technology skills, including use of a mobile device.

Responsibilities

• Assist patients in navigating post-acute care with patient-centered transition plans.
• Incorporate evidence-based guidelines and metrics to promote quality and efficiency.
• Represent area branches in relationships with health systems, hospitals, physicians and executive opportunities.

Education

Graduate of approved nursing/therapy/social work program

RN or PT preferred

Two years of field experience
