Job summary

Enhabit Home Health & Hospice is seeking a qualified professional to guide patients through post-acute care, aiming to improve outcomes and satisfaction. You will develop patient-centered transition plans, integrating evidence-based guidelines and regulatory requirements.

The role emphasizes collaboration with health systems and physicians to foster efficient care delivery. Strong nursing/therapy or social work background and licensure are required, with field experience preferred.

Qualifications

• Education and experience, essential.
• Must be a graduate of an approved school of nursing, therapy or social work.
• Must be licensed in the state where they currently practice.
• Must have two years' demonstrated field experience.
• Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Assists patients in navigating post-acute care to improve outcomes and referral source satisfaction.
• Develops transition plans using evidence-based guidelines to promote quality and efficiency in home health and hospice care.
• Represents the area branches in strategic relationships with health systems, hospitals, and physician groups.
