Job summary

Qualifications

• Bachelor’s degree or equivalent experience preferred.
• Three years’ experience in a health care setting preferred.
• Three years’ customer service experience preferred.
• Excellent communication and organizational skills.

Responsibilities

• Ensures IMM, OBS, MOON letters are delivered timely to meet regulations.
• Schedules PCP appointments for patients at high risk for readmissions.
• Performs discharge paperwork, referrals, follow-up calls, and authorizations.
• Documents work in EPIC including notes and queues.
