Qualifications

MSW required; licensure in Maryland as LMSW or LCSW preferred.
1-3 years hospital/healthcare discharge planning experience.
Ability to educate patients and families about resources and care plans.

Responsibilities

Interview patients and families to assess psychosocial needs.
Facilitate family meetings and develop continuing care plans.
Coordinate complex care plans with home health, social services, and mental health providers.
Educate patients about community resources, advance directives, and end-of-life issues.
