Job summary

Kindred Hospital San Antonio Central is seeking a Care Coordination professional to collaborate with the Interdisciplinary Care Transitions team, manage patient care paths, and optimize resource use throughout the continuum of care.

The role focuses on assessment, care planning, and discharge planning to improve patient outcomes and cost effectiveness while maintaining quality and compliance with review agencies.

Qualifications

• Knowledge of government and non-government payor practices, regulations, standards and reimbursement.
• Knowledge of Medicare benefits and insurance processes and contracts.
• Knowledge of accreditation standards and compliance requirements.
• Must read, write and speak fluent English.
• Basic computer skills with Microsoft Office suite.

Responsibilities

• Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians.
• Assist with care coordination and facilitation.
• Refer high risk patients for additional support as needed.
• Act as a patient advocate and participate in rounds/conferences.
• Collaborate with staff to execute the plan of care and meet goals.
