Job summary

Good Samaritan is seeking a skilled admissions coordinator to identify potential admissions and manage the processing, placement, and status tracking of new resident/client requests. You will handle admission paperwork, insurance verifications, authorizations, and transportation coordination when needed.

You will collaborate with discharge planners, families, and the healthcare team to plan and execute admissions while ensuring regulatory compliance and data accuracy in CRM and EMR systems.

Qualifications

• Bachelor’s degree from an accredited school is preferred but not required.
• Prior work experience in long term care facility preferred.
• Unencumbered license RN, LPN, OT, PT, or LSW with applicable state licensure in practice states.
• Home-based services option may require COTA/PTA licensure with multi-state privileges.
• Maintains competencies and certifications; some travel may be necessary; valid driver’s license may be required; BLS required.

Responsibilities

• Identify potential admissions and process/place new resident/client requests and track admission status.
• Coordinate admission-related tasks, including paperwork, insurance verification, and prior authorizations; manage transportation when applicable.
• Collaborate with discharge planners, family, and the interdisciplinary team to plan, implement, and evaluate admissions; ensure smooth transitions.
• Deliver high-quality customer service; handle confidential information; coordinate multiple responsibilities; provide guidance and training as needed.
