Creative Solutions in Healthcare, Inc. is seeking an Admissions Coordinator to guide residents and families through the admissions process with compassion and professionalism.

You will build relationships with residents, families, and referral sources, review documentation, and collaborate with clinical and administrative teams to ensure smooth transitions and compliant patient care. Travel locally as needed to support referral development and engagement, and represent the facility in community

Qualifications

• Excellent written, verbal, and public speaking skills are required.
• Ability to work independently and manage multiple priorities in a fast-paced environment.
• Genuine passion for serving elderly and disabled individuals.
• Strong organizational and relationship-building abilities.

Responsibilities

• Build strong relationships with residents, families, and referral sources to ensure a smooth admissions process.
• Complete bedside assessments, review documentation, and clinically accept residents for admission.
• Prepare weekly reports and maintain organized records of all admissions activity.
• Collaborate with clinical and administrative teams to support seamless transitions for new residents.
• Represent the facility in community presentations on long-term care, aging, and health services.
• Promote community outreach and strengthen relationships with healthcare partners.
• Travel locally as needed to support referral development and engagement efforts.
• Comply with the Patient Bill of Rights, Employee Responsibilities, and all company safety and policy procedures.
• Demonstrate reliability, professionalism, and teamwork in all interactions.
