Responsibilities

• Identify potential admissions and process new resident requests and track admission status.
• Coordinate admission paperwork, insurance verifications, prior authorizations, and transportation as applicable.
• Collaborate with referral sources and interdisciplinary team to plan, implement, and evaluate admissions.
• Deliver clinical evaluations on resident charts to determine appropriate medical services at accepting facility/agency.
• Educate healthcare team regarding trends, regulations, and internal policies affecting resource utilization.
