Virginia Commonwealth University Health seeks a discharge planner to coordinate post-hospital placements with Medicare/Medicaid and community resources for safe transitions. The role requires independent critical thinking, excellent communication, and computer proficiency. A Bachelor’s Degree in a related Human Services field is required; a Social Work degree is preferred.

Qualifications

• Bachelor’s Degree in related Human Services field from an accredited program; an equivalent combination of education/experience may be considered.
• Two (2+) years of experience in acute care or healthcare setting with emphasis on care management/discharge planning preferred.
