Responsibilities

• Coordinate placement and post-hospital care with community resources.
• Interpret Medicare/Medicaid/Commercial insurance rules for post-acute care.
• Collaborate with Healthcenter staff in a fast-paced environment.
• Assist patients and families with discharge planning and education.
• Maintain accurate documentation and regulatory compliance.
