Memorial Health is seeking a Patient Discharge Specialist to coordinate discharge plans from hospital to extended care, home, or other care levels. The role includes verifying insurance, discussing options with patients and families, and coordinating post-discharge services.

Qualified candidates may have an LPN license and at least two years of acute or long-term care experience. Strong communication, reimbursement knowledge, and office skills are required.

Qualifications

• Licensure/Certification/Registry: LPN preferred but not required.
• Experience: minimum two years in acute or long-term care.
• Understanding of reimbursement, strong communication, customer service, and basic office computer skills.

Responsabilités

• Discharge coordination: collaborate with care teams to arrange discharge plans and choices for facilities or home health.
• Utilization management support: data collection, follow-up on discharge changes, and maintain relationships with providers.
• Participate in on-call and weekend coverage schedules.
