Job summary

Qualifications

• Bachelor's Degree in related Human Services field required or an equivalent combination of education, training and/or experience.
• Two (2)+ years of experience in acute care or healthcare setting preferred (discharge planning/care management).
• Strong oral and written communication, computer proficiency, and ability to work independently.

Responsibilities

• Coordinate discharge planning and post-hospital care with patients, families, and care teams.
• Navigate Medicare/Medicaid/Commercial insurance regulations for placement and services.
• Interact with Healthcenter staff to arrange community resources and services.
