Job summary

MUSC Health is seeking a Program Coordinator – Case Management to support RN Case Managers and Social Workers within the interdisciplinary healthcare team across the continuum. The role coordinates payer communication, transition plan implementation, and the delivery of patient documentation as part of the Case Management department.

The position reports to the Manager of Case Management and involves regular collaboration across departments to ensure smooth transitions and thorough documentation

Qualifications

• Education & Experience: A Bachelor's degree and one year work experience; or high school diploma (or equivalent) and 2 to 4 years work experience.
• Supports the RN Case Managers and Social Workers in the interdisciplinary healthcare team across the continuum.

Responsibilities
