You will engage families, participate in treatment team discussions, and ensure timely hand-offs to community services while maintaining confidentiality and professional standards.

Qualifications

• Bachelor's degree in a related field is required.
• 2 years' experience in the mental health field is required.
• Strong communication and coordination skills are essential.
• Ability to work as part of a multidisciplinary treatment team.
• Knowledge of discharge planning and community resources is preferred.

Responsibilities

• Coordinate discharge and aftercare planning with families and providers.
• Schedule outpatient appointments and medication management, coordinating times.
• Enter discharge planning notes in the EHR.
• Track court report due dates and alert clinicians.
• Participate in treatment team meetings and provide discharge planning resources.
