Qualifications

• Minimum of 2 years of full-time health-care related work experience with exposure to behavioral and medical situations.
• Experience with administrative or operational processes in a health-care setting preferred.

Responsibilities

• Develop individualized discharge plans for patients.
• Schedule all follow-up appointments for ongoing care within 7 days of discharge.
• Coordinate with Utilization Management on all authorizations and continued authorizations.
• Complete initial assessment and treatment plan for each patient.
• Provide therapeutic services, including counseling and discharge planning.
