Job summary

Qualifications

Responsibilities

• Coordinate and facilitate discharge planning with patients, families and care team.
• Assess patient needs and develop individualized care plans to meet goals.
• Ensure timely transitions between acute and post-acute care and appropriate next level.
• Communicate with multidisciplinary team and document care progress.
