Job summary

Hackensack Meridian Health is seeking a Registered Nurse to provide direct patient care within a multidisciplinary team, delivering evidence-based nursing practices and coordinating care across shifts.

You will assess patients, plan and implement care, document findings, educate patients and families, and support safe transitions through effective discharge planning. Requires an associate degree in nursing and an RN license; BSN preferred within specified timeframe.

Qualifications

• Associate degree in Nursing or Diploma required.
• BSN in progress or plan to obtain within three years.
• Six months nursing experience or active participation in Nurse Residency Program at HMH.
• Ability to learn quickly, work in a team, and deliver high-quality patient care.

Responsibilities

• Assess and collect comprehensive data on patient health.
• Plan, implement, and evaluate patient care plans.
• Coordinate care with physicians and interdisciplinary team.
• Provide patient and family education and ensure understanding.
• Administer medications using bar-scanning and accurate documentation.
• Discharge planning to ensure safe transitions.
