Job summary

Hackensack Meridian Health is seeking a Care Coordinator in Care Management to coordinate, communicate, and facilitate the clinical progression and discharge planning for patients. This role involves working with a multidisciplinary team to plan care, ensure smooth transitions, and connect patients to appropriate next-level services.

The position requires a BSN (or in progress) or MSW with strong communication and organizational skills.

Qualifications

• BSN or BSN in progress and/or willing to acquire within 3 years of hire; or Masters Degree in Social Work.
• Master's degree preferred.

Responsibilities

• Assesses patients and develops individualized care plans with the physician and health care team.
• Facilitates communication among health care team and patient/family to minimize fragmentation of services.
• Maintains community resources knowledge and refers patients appropriately.
• Ensures timely and appropriate transitions to next level of care.
• Develops discharge plans with patient and support persons and documents in medical records.
• Participates in committees and rounds; identifies quality issues for review.
• Reassesses progress and updates discharge plans as condition changes.
• Provides patients and families with resources and discharge options and education on benefits.
