Qualifications

• RN license in Alabama with 2 years of experience required.
• BSN and related healthcare bachelor's degree preferred.
• Knowledge of managed care, governmental payers, and third-party reimbursement.
• Strong communication and organizational skills.

Responsibilities

• Assesses patients within 24 hours to determine appropriate care level and discharge needs.
• Collaborates with multidisciplinary teams to coordinate care and reduce readmissions.
• Monitors tests, procedures, and resources to ensure efficient care transitions.
• Facilitates care conferences and communicates plans to providers and families.
• Supports guardianship, palliative involvement, and legal documentation as needed.
