Job summary

Providence at Home with Compassus in California is seeking a Clinical Care Partner I to coordinate safe, patient-centered transitions of care from hospital to home-based services.

This in-person role requires bedside engagement, collaboration with physicians and case management, and participation in discharge planning and post-acute care coordination. You will help assess patients, arrange home health, hospice, and other services, educate families, and support value-based care goals.

Qualifications

• Required: LPN/LVN or equivalent designation with CPR certification.
• Preferred: Bachelor's degree in Nursing or Health Sciences.
• Experience: 2–3 years in care coordination, discharge planning, or healthcare services.
• Experience with EMR systems (Epic) and referral platforms.

Responsibilities

• Coordinate safe hospital-to-home transitions and post-acute care arrangements.
• Evaluate patients for post-acute care eligibility and services.
• Collaborate with physicians and care teams for level-of-care decisions.
• Coordinate post-acute services and patient handoffs to providers.
• Educate patients and families about post-acute options and care expectations.
• Maintain EMR documentation for care coordination activities.
