Qualifications

Responsibilities

• Coordinate safe, efficient transitions of care for hospitalized patients.
• Evaluate patients for home-based and post-acute care services.
• Order and arrange post-acute services including home health, hospice, and equipment.
• Provide bedside education to patients and families about post-acute care options.
• Collaborate with physicians, case management, and discharge planners to support timely discharges.
