Region VII Area Agency on Aging seeks a CHW Care Attendant/Driver to support discharged patients by coaching, coordinating with hospitals, and providing transportation to appointments. The role includes home visits, preparation assistance, and helping clients settle back at home after visits.

Qualifications include a HS diploma, CHW certification (or willingness to obtain within 3 months), CNA preferred, and a valid driver's license.

Competenze

• Community Health Worker certification required; willing to obtain if not yet certified within three months.
• Certified Nursing Assistant certification strongly preferred.
• Valid driver's license with chauffeurs endorsement or willingness to obtain within three months.
• Experience in community-based settings and working with diverse populations is preferred.
• Ability to drive, travel to residential appointments, and assist with transportation.

Mansioni

• Use care transition model to reduce readmissions and coordinate with hospital partners.
• Provide transportation to appointments and assist with visit preparation.
• Conduct home visits and help participants with basic needs for appointments.
• Coach patients on chronic health management and access to community resources.
• Document activities, service plans, and outcomes in accordance with policies.
