Job summary

CenterWell Home Health is seeking a Home Health Aide to support patients in their homes under the supervision of the Associate Director, Branch Administration. You will provide direct patient care, assist with activities of daily living, monitor vital signs, help with medications and mobility, document care provided, and maintain a clean, safe home environment.

The role requires CNA or HHA certification within the last 24 months, a valid driver’s license, state certification, and driving

Qualifications

• Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
• Must meet applicable state certification requirements.
• A valid driver's license, auto insurance, and reliable transportation are required.
• Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
• Minimum one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.

Responsibilities

• Provide direct patient care to patient under the direction of the RN and according to the Aide Plan of Care (POC).
• Assist the patient with self-administered medications by reading medication labels and observing proper dosage.
• Take accurate vital signs and report abnormal results to supervisor.
• Help patient maintain personal hygiene and assist with bathing, grooming, and oral care.
• Assist with feeding and monitor nutrition and intake.
• Assist with toileting and catheter/ostomy care as needed.
• Provide skills to safely assist with mobility, transfers and ambulation as per Plan of Care.
• Document interactions with patients and caregivers thoroughly and timely.
• Practice infection control and maintain a clean, safe home environment.
• Assist with light household tasks and meal prep when needed.
