Job summary

Region VII Area Agency on Aging is seeking a CHW Care Attendant/Driver to support patients after discharge by coaching, coordinating with hospitals, and providing transportation to appointments. The role emphasizes empowering patients in their post-discharge recovery and linking them to community resources.

The position requires a CHW certification, a valid driver’s license, and the ability to travel to residences and healthcare facilities.

Qualifications

• High school diploma or equivalent with 1–2 years in a community-based setting preferred.
• CHW certification required; if not certified, obtain within 3 months of hire.
• Certified Nursing Assistant certification strongly preferred.
• Valid driver’s license with chauffeur endorsement or ability to obtain within 3 months of hire.
• Desirable experience: customer service, patient coaching, medical interactions, outreach, data entry, public speaking, and non-profits.

Responsibilities

• Actively participate in team meetings, sharing insights and reviewing readmission trends.
• Develop and maintain open communication with care transition hospital partners.
• Work on-site at hospitals following care transition policies.
• Implement the Community Care Transition Initiative model with target outcomes.
• Conduct home visits and assist with appointment preparation.
• Provide transportation to appointments and assist participants during visits.
• Accompany participants to appointments to assist with note taking and instructions.
• Ensure participants have needed supplies after appointments and resupply as needed.
• Coach patients in managing chronic conditions and accessing community resources.
• Document activities and outcomes per policies; travel to residential appointments as required.
