Job summary

Trustbridge Hospice, a member of Empath Health, seeks a dedicated Care Team Manager in Fort Lauderdale to lead the Interdisciplinary Group and elevate patient and family care during advanced illness.

As an RN Manager, you will provide clinical leadership, guide the IDG, mentor staff, ensure quality documentation, and drive continuous improvement. Join a mission-driven team committed to compassionate, Full Life Care for All.

Qualifications

• Registered Nurse licensed in Florida.
• Current Basic Life Support Certification.
• One year of recent leadership experience in hospice, oncology, medical/surgical, or home health, public/health, geriatric care experience preferred.
• Experience with clinical documentation and EMR is a plus.

Responsibilities

• Provide clinical leadership to guide patient care.
• Lead the interdisciplinary team (IDG) and coordinate collaboration among physicians, nurses, and field staff.
• Drive excellence in care delivery with individualized, patient-centered care plans.
• Develop and mentor staff; training, coaching, and performance evaluation.
