Synergy Home Care is seeking a versatile Care Services Coordinator & Community Liaison to supervise in-home care delivery and expand our referral network in the Chicago area. The role combines field supervision with community outreach to strengthen client relationships and grow our client base.

The position includes participation in after-hours on-call rotations, requires valid driver's license and reliable transportation, and offers competitive pay with mileage reimbursement.

Qualifications

• Experience in supervising staff or teams in home care or healthcare settings.
• Strong outreach/marketing capabilities to build referral networks.
• Excellent communication and problem-solving skills; ability to manage multiple stakeholders.

Responsibilities

• Oversee care supervision and field operations to ensure quality and client satisfaction.
• Build and maintain referral relationships with hospitals, physicians, and community partners.
• Conduct home visits, provide coaching to caregivers, and support onboarding and training.
