Job summary

HomeWell Care Services, Inc. is seeking a Care Manager to coordinate, implement, and supervise the delivery of services to agency clients in alignment with our mission and standards of care.

This position is accountable for adherence to regulatory and agency requirements for service provision and ensures compliance with and maintenance of client records. The Care Manager will travel to client residences, develop written care plans with input from clients or designated representatives, supervise

Qualifications

• Two years of experience in home care, home health or related field.
• A reliable means of transportation with current insurance and a valid driver’s license.
• Excellent written and verbal communication skills with diverse clients, families, and professionals.
• Impeccable follow up, attention to detail, initiative and sound judgment.
• Able to supervise, assess and evaluate care providers and advocate for clients and families.
• Passionate about helping others live with dignity.
• Must present a positive, professional business image.
• Must pass a criminal background check.

Responsibilities

• Complete Client Assessment packet for each new client and provide copies of signed agreements.
• Develop and implement a written care plan for each client with input from the client or designated representative.
• Include client functional limitations, nutritional needs, equipment, services, frequency, and non-medical tasks.
• Collect fees for initial services at assessment or first day of service.
• Provide caregiver introduction, review care plan with client and caregiver, and set expectations for service delivery.
• Supervise client care to ensure caregivers follow the care plan and maintain infection control practices.
• Monitor changes in client condition and service needs and report promptly.
• Coordinate with other providers when multiple services are involved.
• Facilitate client education, empowerment, and referrals as needed.
• Document supervision and maintain current client records per policy.
• Provide training to staff in the client’s home or at the agency.
• Communicate with clients or their representatives to ensure satisfaction.
• Assist with policy development and participate in caregiver evaluations.
