HomeWell Care Services CT340 in Stamford, CT is seeking a Care Manager to coordinate, implement, and supervise the delivery of services for agency clients. You will ensure adherence to regulatory requirements and maintain accurate client records while leading a team of caregivers.

The role requires supervisory skills, strong communication, and a passion for helping others live with dignity. Travel to client residences is expected as part of the job.

Qualifications

• Two years of experience in home care, home health or related field.
• A reliable means of transportation with current insurance and a valid driver’s license.
• Excellent written and verbal communication skills and be able to interact with clients, families, care providers and other professionals.
• Impeccable follow up and attention to detail; ability to exercise initiative and problem solve.
• Able to supervise, assess and evaluate care providers and advocate on behalf of clients and families.
• Passionate about helping others live with dignity.
• Must present a positive and professional business image.
• Must pass a criminal background check.

Responsibilities

• Complete Client Assessment packet for each new client and provide Signed agreements to client or designated representative.
• Develop and implement a written care plan for each client with input/approval by client or designated representative.
• Collect fees for initial services at the time of assessment or by the first day of service.
• Provide caregiver introduction for first scheduled shift; review care plan with client and caregiver and set expectations for service delivery.
• Supervise client care to ensure caregivers follow the care plan and use proper infection control practices.
• Regularly communicate with the client or designated representative to follow-up on service delivery and satisfaction.
• Perform scheduled and non-scheduled in-home client support visits.
• Conduct reassessments according to the agency’s frequency and changes in client condition.
• Coordinate services when more than one agency is providing care.
• Serve as a resource for clients and families, provide education and referrals as needed.
• Investigate issues related to client care and wellbeing; document supervision and maintain records.
