HomeWell Care Services – Augusta is seeking a dedicated Client Care Manager to oversee the client experience from inquiry through discharge. You will coordinate personalized care plans, supervise caregivers, manage scheduling, maintain compliance, and build trusted relationships with clients, families, and referral partners.

In addition to care management, you will drive client acquisition and retention, converting inquiries into starts, strengthening referral networks, and supporting agency

Qualifications

• At least 3 years in non-medical home care services required.
