HomeWell Senior Care, Inc. in Stamford, CT is seeking a Care Manager to coordinate, implement, and supervise the delivery of services for agency clients. You will ensure adherence to regulatory standards and maintain client records, reporting to the Operating Manager / Owner.

The role requires strong communication, problem-solving, and leadership to supervise caregivers and advocate for clients and families. Benefits include 401(k) matching, bonus, PTO, and training opportunities.

Qualificações

• Minimum two years of home care experience.
• Valid driver’s license and reliable transportation.
• Excellent written and verbal communication.
• Ability to supervise and evaluate care providers.
• Advocate for clients and families.
• Pass criminal background check.
• Positive, professional business image.

Responsabilidades

• Complete Client Assessment packet for each new client.
• Develop and implement a written care plan with input from client or designated representative.
• Collect fees for initial services at assessment or first day of service.
• Provide caregiver introduction and review care plan with client and caregiver.
• Supervise client care to ensure adherence to care plan and safety.
• Regularly communicate with client or designate client representative.
• Conduct in-home support visits and reassessments as needed.
• Coordinate with other services to ensure comprehensive care.
• Document supervision and maintain client records according to policy.
• Provide training to staff in client’s home and via office.
