Job summary

A First Name Basis Home Care is seeking a dedicated Care Manager to develop personalized care plans, facilitate access to resources, and ensure the well-being of clients through supervision and intervention strategies.

You will supervise staff, conduct assessments, coordinate services with community resources, and maintain accurate documentation in line with organizational policies. Requires 2+ years in homecare; high school diploma; reliable vehicle for travel within the area.

Qualifications

• 2+ years of experience in homecare preferred.
• High school diploma required; Bachelor’s degree preferred or equivalent work experience.
• Office/administrative support skills.
• Excellent written and verbal communication skills.
• Reliable vehicle for travel within area.
• Ability to work in a fast paced, multi-task environment with competing priorities.

Responsibilities

• Develop and implement individualized care plans tailored to the unique needs of each client.
