HomeWell Care Services, Inc. is seeking a Care Manager to coordinate, implement, and supervise in-home services for agency clients, aligning with our mission and standards of care.

The role ensures compliance with regulatory requirements and accurate maintenance of client records, while collaborating with agency leadership to uphold cultural and business standards and empower caregivers to deliver best-in-class care tailored to each client’s needs.

Responsibilities

• Coordinate, implement and supervise the delivery of services to agency clients.
• Ensure adherence to regulatory and agency requirements for service provision and client records.
• Work with HomeWell Agency leadership to align standards and empower the Caregiver to provide best-in-class in-home care prioritized to client needs.

The Care Manager is responsible for coordinating, implementing, and supervising the delivery of services to agency clients in alignment with the company’s mission and standards of care. This position is accountable for adherence to regulatory and agency requirements for service provision and ensures compliance with and maintenance of client records.

The Care Manager will work closely with HomeWell Agency leadership to develop and ensure cultural, organizational and business standards are aligned and empower the Caregiver to administer best in class in-home care where individual client needs are prioritized and accomodated.
