Job summary

InnovAge Holding Corp. is seeking a Care Plan Coordinator to oversee the development and completion of participant care plans in alignment with federal and state regulations and InnovAge policies.

The role includes coordinating interdisciplinary discussions, ensuring timely care plan audits, and promoting accurate documentation across care teams. Strong nursing knowledge, CPR, and 3+ years healthcare experience are required.

Qualifications

• Active LPN license to practice in New Mexico.
• Graduate of an accredited Medical Assistant program.
• Current CPR certification or ability to obtain CPR certification.
• 3+ years of experience in a health care setting.
• Knowledgeable of nursing and medical practices, laws, regulations and guidelines that pertain to PACE.
• Excellent computer technology including e-mail use.
• Proven written and oral communication skills with strong analytical and decision making skills.
• Must be capable of maintaining regular attendance.

Responsibilities

• Oversee and coordinate the development and completion of the participant plan of care in accordance with Federal and State regulations and InnovAge policy.
• Track completed care plans for compliance and maintain the Care Plan schedule spreadsheet.
• Facilitate Care Plan discussions for the Interdisciplinary Team to ensure documentation aligns with care plans.
• Oversee assessment and care plan processes for timeliness and compliance.
• Participate in center committees for quality assessment and assurance as required.
