InnovAge Holding Corp. is seeking a Care Plan Coordinator to oversee the development and completion of participant care plans in alignment with federal and state regulations and InnovAge policies.
The role includes coordinating interdisciplinary discussions, ensuring timely care plan audits, and promoting accurate documentation across care teams. Strong nursing knowledge, CPR, and 3+ years healthcare experience are required.
The Care Plan Coordinator oversees and coordinate the development and completion of the participant plan of care in accordance with current Federal and State regulations, and guidelines that govern the participant assessment, to include the policy and procedures of InnovAge.

The pay may vary depending on job related factors, such as work location, experience, knowledge, skills, education, certifications, training and internal equity.
