Job summary

High Desert PACE is seeking a Care Plan Coordinator to oversee the full lifecycle of care plan activities. You will supervise interdisciplinary team members, coordinate care planning meetings, and drive timely, compliant care delivery for participants.

You will develop and enforce policies, ensure documentation and activation of care plans, and maintain strong communication with staff and departments. This role requires leadership, organizational skills, and a focus on quality care.

Qualifications

• Minimum of two (2) years of experience in a supervisory or clinical administrative role in a health care delivery setting.
• Minimum of one (1) year of documented experience working with a frail or elderly population.
• Must be medically cleared for communicable diseases and up to date immunizations.

Responsibilities

• Participate as a member of the interdisciplinary team and lead Care Planning meetings.
• Oversee Care Planning operations and ensure routine duties meet participant needs.
• Coordinate with HDP departments and communicate with staff across roles.
• Develop, oversee, implement, and adhere to Care Planning policies and standards.
• Meet Care Planning deadlines to maintain compliance and monitor logs.
• Provide bi-monthly status reports to the Chief Clinical Officer.
• Maintain confidentiality of participant and staff information.
• Attend staff meetings, trainings, and committees as assigned.
