Job summary

Carlton Senior Living in Concord, CA is seeking a dedicated Care Partner to assist residents with daily living activities, such as grooming, mobility support, and meal assistance, in a warm, respectful environment. You will join a small care team on an evening shift (2:30 PM–10:45 PM) with on‑the‑job training and clear care plans, plus paths for career growth and advancement.

Compensation ranges from $18 to $19 per hour, with comprehensive benefits and paid time off.

Qualifications

• Must be at least eighteen (18) years old per Title 22.
• Prior caregiving experience encouraged but training provided.

Responsibilities

• Assist residents with activities of daily living, including grooming and meals.
• Monitor and report changes in resident care plans to management.
• Document care using company devices and follow care plans.
• Support safety protocols and elopement prevention.
• Foster a warm, collaborative atmosphere with residents, families, and teammates.

What we are hiring for:

Schedule:

Full Time | Friday through Monday 2:30PM - 10:45PM

Pay Range:

$18.00-$19.00

What You’ll Do:

• Work on a team of Care Partners to assist our resident with activities of daily living that can include continence care, showering, grooming, transfer assistance, and meal assistance.
• Monitor and report changes in resident care plans to management.
• Document care accurately using company-provided devices and follow individual care plans.
• Support safety protocols, emergency plans, and elopement prevention practices.
• Foster a warm, collaborative atmosphere with residents, families, and team members.

Qualifications:

• Must be at least eighteen (18) years or older, per Title 22 requirements
• Prior caregiving experience preferred but not required – we will train the right candidate!
