Job summary

The Elizabeth Layton Center in Ottawa, KS is seeking a dedicated Care Coordinator to join our Quality Improvement team. This role focuses on coordinating whole-person care, including behavioral and physical health, and engaging clients with internal and external partners.

You will facilitate screenings, referrals, and documentation while supporting a collaborative, inclusive work environment. Responsibilities include welcoming clients with outstanding service, guiding transitions across care

Qualifications

• High school diploma or equivalent; some college preferred.
• Minimum 3 years of clerical or general office experience; medical office experience a plus.
• Compassionate attitude and professional demeanor.

Responsibilities

• Welcomes individuals to ELC with exceptional customer service and explains services.
• Assists individuals in transitions to different types/levels of care.
• Completes required documentation and health measures (blood pressure, height, weight).
• Performs data entry and monitors records in Excel and EHR systems.
• Coordinates care among providers and follows up as needed.
• Maintains confidentiality of all client information.
• Completes trainings and adheres to ELC policies.
