Job summary

WelbeHealth, LLC. seeks a Community Health Worker I to support PACE participants at home and in the community, addressing SDoH needs and connecting seniors to essential services.

You will work with an interdisciplinary IDT to coordinate resources, assist with program applications, and help participants access housing, food, transportation, and caregiver supports, while promoting health, safety, and stability.

Qualifications

• High school diploma or equivalent.
• Associate’s degree preferred in social work, human services, social science, or relevant field.
• Knowledge of community-based services and resource navigation.
• Experience with older adults or vulnerable populations is a plus.
• Ability to work independently with minimal supervision.

Responsibilities

• Provide direct participant support to address SDoH needs through home visits and outreach.
• Connect participants to established community resources and verify eligibility for services.
• Assist participants with completing and submitting applications for community resources.
• Support Social Workers in discharge planning with resource information during PACE disenrollment.
• Support participants with complex needs through ongoing engagement and barrier resolution.
• Collaborate with Social Workers, ECAs, PELs, and the IDT to reinforce participant-centered care plans.
