Denver Health is seeking a Care Navigator to provide comprehensive patient navigation services, coordinating referrals and ensuring care continuity across inpatient and outpatient settings. The role focuses on ambulatory and HIV care, discharge planning, follow-up, and connecting patients with home and community resources.

The navigator will support patients and families in navigating the health system, addressing barriers, and improving engagement in care while collaborating with the care team

Formación

• 1-3 years experience in a clinical care setting.
• Strong communication and empathy with diverse populations.
• Bilingual English/Spanish preferred.
• Knowledge of Denver Metro area community resources.
• Familiar with Epic and MS Office.

Responsabilidades

• Manages transitions of care across settings (ED, inpatient, SNF, specialty to primary care).
• Addresses barriers to facilitate care coordination and retention in care as required by grant funders.
• Maintains documentation on referrals to promote team awareness and patient safety.
• Assembles information on patient background and referral needs per guidelines.
