Job summary

The Care Coordinator at NTACHC collaborates with clinicians to manage patient referrals and treatment plans. You will arrange specialist visits, verify insurance, and ensure proper documentation in the NextGen EMR, maintaining HIPAA compliance and patient confidentiality.

This role emphasizes timely follow-ups, cross-clinic communication, and adherence to care coordination standards to improve patient outcomes and service quality.

Qualifications

• High school diploma or GED required.
• Bilingual English/Spanish preferred.
• Medical Assistant Certificate preferred.
• Valid driver’s license required.
• Prior Medical Assistant experience a plus.

Responsibilities

• Coordinate referrals and appointment arrangements for patients.
• Verify patient insurance with partner institutions.
• Follow up with patients to ensure referrals and notes are obtained.
• Document all patient information accurately in the EMR.
• Communicate with clinics, specialists, and management.
• Support performance improvement initiatives and HIPAA compliance.
