Job summary

Generation-Gaither seeks a Care Manager to support individuals in navigating community resources and coordinating care across health providers. The role emphasizes empowering clients through personalized planning and timely documentation.

The Care Manager will collaborate with the Care Team to implement Comprehensive Care Plans, Crisis Plans, and ongoing assessments while adhering to state and federal regulations. Training and documentation duties are also required.

Qualifications

• Bachelor’s degree in health related field or must be licensed nurse.
• LPN’s must have fifteen (15) semester hours of behavioral health and one (1) year of behavioral health care experience.
• Alternate Bachelor’s degree must have fifteen (15) college level semester hours of coursework in behavioral health or at least one (1) year of work experience in a behavioral health setting.

Responsibilities

• Provide care management services to members to access community resources and develop a person-centered care plan.
• Coordinate with care team and various providers to ensure timely documentation and service delivery.
• Develop and review Comprehensive Care Plans and Crisis Plans within required timelines.
• Ensure documentation meets regulatory and organizational standards and include all required elements.
