Job summary

Sun River Health is seeking a Full-time Patient Navigator to support our Washington St. - Poughkeepsie, NY site.

The navigator identifies individuals in need of preventive and primary health care services and links them to the appropriate care pathways. The role supports a wide range of services including acute and chronic disease care, dental, prenatal and family planning, WIC, nutrition services and pediatric immunizations.

Qualifications

• Minimum Education Requirement: High school diploma/GED.
• Minimum Work-Related Experience: 0-2 years.
• Bilingual Spanish is preferred.

Responsibilities

• Identify individuals and/or family members within the target population who need preventive and primary health care services and link them to the required services.
• Provide care for acute, chronic and communicable diseases; dental services; prenatal care and family planning; WIC and other nutrition services; pediatric care and immunizations.
• Trains and mentors the Patient Representatives. Perform other duties as assigned on an as needed basis.
