Job summary

Universal-Community-Health-Center seeks a Referral Coordinator to manage patient referrals, coordinate care with providers, and verify insurance for timely access to essential services. Qualifications

• High School or equivalent required.
• Current Medical Assistant certificate or one year of Referral Coordinator experience may substitute for the Medical Assistant certificate.
• Excellent verbal and written communication skills.

Responsibilities

• Coordinate and process medical referrals for services not provided within the clinic.
• Schedule specialty care referrals and communicate with external providers.
• Obtain, track, and manage pre-authorizations per payer requirements.
• Maintain accurate referral documentation and electronic patient tracking.
• Provide appointment reminders and ensure copies of referrals are given to patients.
• Coordinate pre-operative orders and schedule required appointments.
• Monitor referral status and follow up with patients, providers, and payers to ensure completion of services.
• Ensure compliance with payer guidelines and regulatory requirements.
• Serve as liaison between patients, providers, and external partners.
• Identify and address barriers to care to support patient navigation through referrals.
