Job summary

Suncoast Hospice, part of Empath Health, seeks a Care Navigator (Customer Service Specialist) to coordinate service inquiries and provide prompt access within the Empath Health Network. You will triage inquiries, facilitate appointments, and input referrals into the EMR with care and accuracy.

You will interact with patients, families and providers, maintaining confidentiality and delivering empathetic service while supporting the continuum of care.

Qualifications

• High school diploma or GED or equivalent experience.
• Excellent communication with patients, families and colleagues.
• Bilingual preferred.
• Strong multitasking and critical thinking.
• Knowledge of Microsoft Office; HIPAA compliance.
• Basic healthcare terminology and community resources knowledge.

Responsibilities

• Provides optimum access to Empath Health Care Continuum via telephone, website, fax and walk-in inquiries.
• Facilitates appointments through effective triage to the appropriate Care Navigation program.
• Inputs referrals into the EMR accurately and in real time.
• Works with patients, families and providers to ensure complete information for referrals.
• Maintains confidentiality with empathetic, compassionate service.
