Job summary

Suncoast Hospice, part of Empath Health, seeks an RN Case Manager to oversee patient care across home and facility settings in Pinellas County. Typical schedule is Monday through Friday, 8a-5p with occasional weekend days and seasonal holidays.

You will review care plans, educate patients and families, and coordinate care with the care team and physicians to ensure compassionate, full-life care for all.

Qualifications

• Graduate of an approved nursing program and hold a Florida RN license.
• Minimum one year of acute-care hospital experience providing direct patient care; hospice experience preferred.
• Valid Florida driver's license (as needed for travel between facilities).

Responsibilities

• Reviews and updates the care plan, physician orders, pharmacy profile and other records.
• Educates patients, families, and caregivers about the disease process, palliative options, and safety practices.
• Coordinate and deliver care across home and facility settings within Pinellas County.
