Nationwide Children’s Hospital seeks a Case Management Extender to navigate services and support care coordination for patients and families. This role connects families with community resources and local agencies to address social determinants of health, across inpatient stays, clinic visits, ER visits, and home or community settings.

Responsibilities include assisting with family-centered care development, maintaining records, and collaborating with PCPs, specialists, and home care providers

Qualifications

• Bachelor’s degree in health care, public health, or related clinical field preferred.
• Excellent communication and interpersonal skills to work with families and providers.
• Experience in patient-facing healthcare roles is beneficial.

Responsabilités

• Engage and motivate members to participate in case management programs.
• Provide navigation support to connect families to community resources.
• Coordinate with healthcare teams and external agencies to address social determinants of health.
• Maintain and organize case records with accurate documentation.
• Answer calls, schedule appointments, and assist with real-time problem solving.
