Job summary

New Mexico Solutions is seeking a Care Coordinator for the CareLink Program to support individuals with behavioral health conditions and complex medical needs. You will coordinate referrals, help members navigate Medicaid MCOs, and maintain monthly contact to promote progress on care plans.

Qualifications include a Master’s or Bachelor’s in a human service field, experience with Medicaid, and strong communication skills. Bilingual English/Spanish is preferred but not required.

• Master’s degree or relevant experience in behavioral health care.
• Experience navigating Medicaid MCOs and healthcare benefits.
• Ability to conduct comprehensive needs assessments and care planning.
• Strong documentation and time-management skills.

Responsibilities

• Coordinate specialty medical referrals and follow up on access to services.
• Assist members with navigating Medicaid MCOs and community resources.
• Maintain monthly contact to monitor progress toward care plan goals.
• Complete annual Comprehensive Needs Assessments and update care plans.
