Job summary

Nationwide Children's Hospital in Columbus, OH seeks a Case Management Extender for the Partners for Kids program to navigate patients to community resources and coordinate care across inpatient, outpatient, and home settings. You will work with families to identify services and foster relationships with local agencies and schools.

Join a multidisciplinary team to educate families, support care transitions, and promote population health initiatives. 40 hours weekly, full-time, benefits eligible.

Qualifications

• Bachelor's degree in health care, public health, or related field preferred.
• Experience in healthcare and patient-facing roles preferred.
• BLS certification required.
• Valid Ohio driver’s license and auto insurance required.

Responsibilities

• Engages and motivates members to participate in case management programs.
• Assists with development of family-centered care.
• Collects information for tailored assessments and referrals.
• Facilitates communication among the healthcare team.
• Provides navigation support to move patients through the system.
• Connects families to resources addressing social determinants of health.
