Job summary

Your Health Organization in Loris, South Carolina, is seeking a Clinical Scheduler to support the Care Manager team. The role coordinates all in-person visits and provider encounters to ensure timely, appropriate care within our value-based model.

Based in the Loris office, the full-time schedule runs 8:00 AM–5:00 PM, Monday–Friday, with exposure to multiple scheduling systems to improve efficiency and patient outcomes.

Qualifications

• Minimum high school diploma or equivalent required; healthcare training preferred.
• Experience in medical scheduling or administrative roles is desirable.
• Familiar with medical terminology and insurance verification where applicable.
• Strong organization and ability to multitask in a fast-paced environment.

Responsibilities

• Register and verify patients and schedule appointments according to the care model.
• Track new patients and ensure follow-up scheduling until completed.
• Coordinate appointments with providers based on availability and urgency.
• Optimize schedules across multiple disciplines within the care team.
• Support discharge planning scheduling for post-acute patients.
• Utilize multiple software systems to track follow-up and outcomes.
• Communicate with patients via email, portal, phone, and in person to gather information and provide guidance.
